I would like to know answers for following questions.
1. Is your Tgt_StagingArea is a Flat File ?
2. If 'Yes' have you enabled 'output column names' to True ?
If 'Yes' this explains why you got addition record in your output rather than input.

Here is the answers
1. Is your Tgt_StagingArea is a Flat File ? Yes
2. If 'Yes' have you enabled 'output column names' to True ? No
Also I checked this Flat File on Unix, there is one record (NOT COLUMN NAME)

1. How you are generating the report?
2. posted logs are complete?
3. What is the value of environment variable APT_NO_JOB_MON?

Seems to me like its generating report for wrong instance or datastage is facing some problem in reading it.
The job started at 02:05:40 in report its 02:05:38.
first check whether its happening for other instances too.
