target Format

Hi all,
Please give the solution for my doubt.
If iam having the sourceformat like
SRepresent jan feb march
1 120 130 120
2 100 900 500
3 300 400 500
I want target in the folloring format
SRep Month Sales
1 Jan 120
1 feb 130
1 march 120 etc..
Please let me know how i can do this.
